These are the most common forms used by Zen for its passengers.

• Zen Travels Credit Card form:  Once a ticket is issued, all customers are required to complete to form and send it by fax at (202) 207-9550

• Passenger Memo for travelers unable to speak English:  This form should be completed and attached with the tickets.
